Skilled Nursing Facilities (SNFs)

Licensed healthcare facilities providing 24/7 nursing care, rehabilitation services, and medical oversight. SNFs are reimbursed through Medicare, Medicaid, and managed care programs and are commonly financed through FHA/HUD, banks, and structured debt.

Assisted Living Facilities

Residential communities offering housing, meals, and assistance with activities of daily living for seniors who do not require full skilled nursing care. Revenues are primarily private pay, with some Medicaid support depending on the state.

Intermediate Care Facilities (ICFs)

Facilities providing long-term care and supervision for individuals with disabilities or chronic conditions who require ongoing assistance but not acute medical treatment. These assets often involve specialized licensing and Medicaid reimbursement structures.

Board and Care Homes

Smaller residential healthcare properties offering room, board, and limited personal care services. These facilities typically serve seniors or special-needs populations and are often privately operated with stable, community-based demand.

Memory Care

Specialized senior housing designed to serve residents with Alzheimer’s disease and other forms of dementia. Memory care facilities require enhanced staffing, secure environments, and tailored programming, supporting premium pricing and differentiated underwriting.

Independent Living

Age-restricted residential communities for seniors who live independently and do not require daily medical care. These assets are primarily private pay and often resemble multifamily housing with hospitality-style amenities.

Substance Abuse & Behavioral Health Facilities

Facilities providing short-term (detox) and long-term (inpatient) treatment for substance use disorders, mental health conditions, and co-occurring diagnoses. These facilities may operate as inpatient, outpatient, or hybrid models, and can include on-site residential housing components or affiliated supportive housing, depending on the level of care and licensing structure.

Ground-Up Construction & Rehabilitation

New development or substantial renovation of healthcare facilities. These projects often require layered capital stacks and specialized lender coordination.

FHA / HUD-Insured Loans

Long-term, fixed-rate, non-recourse financing programs offered through HUD Section 232 for eligible healthcare properties, including skilled nursing facilities, assisted living facilities, board and care homes, and memory care communities. These loans offer high leverage, long amortization periods, and interest rate stability, making them well suited for acquisitions, refinances, renovations, and select new construction or substantial rehabilitation projects.

Nationwide & Local Bank Financing

Conventional bank loans used for acquisitions, refinances, construction, and working capital. Bank financing can offer flexibility, faster execution, and relationship-driven structuring for healthcare operators.

Bridge Debt

Short-term, transitional financing used to acquire, reposition, stabilize, or prepare healthcare assets for permanent takeout financing such as HUD.

Mezzanine Debt & Preferred Equity

Structured capital solutions that sit behind senior debt and provide additional leverage. Often used to fill capital gaps, support acquisitions, or fund expansion while preserving sponsor equity.

Working Capital & Accounts Receivable Financing

Operating capital solutions secured by receivables, Medicaid/Medicare billings, or cash flow. These facilities help operators manage timing mismatches in reimbursement and support day-to-day operations.

PACE Financing

Property Assessed Clean Energy (PACE) financing used to fund eligible energy-efficient and sustainability improvements. PACE can be layered with senior debt and is repaid through a long-term assessment tied to the property.
